Health & Enrollment

This form will need to be completed for all new and returning campers (unless you are currently enrolled in this school year). Please make sure that you fill in each line. You may put “same as above” or a line if the information is the same or does not apply.

Bugspray & Sunscreen

The Bugspray and Sunscreen form allows our staff to apply these products as needed during the camp day.  We have products on hand but you are welcome to send your own if you prefer.

Photo Release

This form will allow us to send pictures and updates through the Lillio app. We do post pictures of our fun activities to our Facebook and Instagram pages as well but do not ever include names. If you prefer to only have your child’s picture sent through Lillio (only visible to the parents in your specific group) please specify Lillio only on the form.

Medical Care Plan and Request for Administration of Medication

If your child has a medical condition or allergy that will require special attention from our staff you will need to complete the Medical Care Plan. If your child will require medication to be kept onsite you will need to complete both the Medical Care Plan and The Request for Medication. Please reach out if you will be sending medication as these forms will require additional signatures.
